FMCSA releases updated medical forms

The new forms come two weeks after earlier versions expired

New versions of the Medical Examiner’s Certificate and Medical Examination Report Form have been released by FMCSA. The new versions have an expiration date of 12/31/2024 in the upper-right corner, which was the only change to the forms.

In a statement issued on December 15, the FMCSA said medical examiners may continue to use the older version of the forms, with an OMB expiration date of 11/30/2021, until stocks have been depleted. However, some states may choose not to accept “expired” medical certificates from CDL-holding drivers. In order to reduce the risk of drivers’ medical cards being rejected by the state, medical examiners should make the change to the new forms as soon as possible.

FMCSA medical forms with a new 12/31/2024 OMB expiration date include:

  • The Medical Examination Report Form (MCSA-5875),
  • The Medical Examiner’s Certificate (MCSA-5876), and
  • The Insulin-Treated Diabetes Mellitus Assessment Form (MCSA-5870).
