The Non-Hazardous Waste Manifest is not an EPA-required
form and is not allowed in certain states. Please check with your state regulatory
agency to verify that the form is appropriate for shipment of non-hazardous waste in
your state.
These forms help reduce the chance of non-hazardous waste materials being mistaken for hazardous waste. Each of the carbonless, 5- or 6-ply forms is available in snap-out and pin-feed formats. The snap-out versions measure 8-1/2" x 11-3/4"; the pin-feed versions measure 9-1/2" x 11".
