Since September 5, 2006, anyone transporting hazardous waste must use the Uniform Hazardous Waste Manifest form. J. J. Keller is an EPA-approved registered printer for these forms.

Manifests are available in 6-ply, carbonless format. Choice of individual snap-out format or continuous format designed for use with pin-feed printers (8-1/2" x 11" when detached).

Continuation sheet also available for use when more than two transporters are needed to carry a waste shipment, or to continue listings started on the manifest.

Each manifest is prenumbered with the Manifest Tracking Number (MTN) and features red marginal words.
